2nd Grade Davanni's Night - February 5 (Sign Up Included)

I give permission for my child __________________________________ to help at Davanni’s Night for Glendale on February 5, 2013.  As only 6 children can help at each shift, I have marked my child’s first and second choice of shifts that they will be available to help with. 

            _______ 4:00-4:45 p.m.                                             ________6:15-7:00 p.m.

            _______ 4:45-5:30 p.m.                                              ________7:00-7:45 p.m.

            _______ 5:30-6:15 p.m.                                             

_______ I would be happy to help chaperone the students while they are working in the kitchen and bussing tables at Davanni’s.  At least one chaperone is needed for each shift.
